- 50% Area Eligibility List - More USDA Guidance to follow
- Corrective Action Procedure - Must be completed and uploaded to application
- Identification of Eligibility Attestation
- Free Meal Policy Statement - Must be filled out if Charging for meals & attached to application
- Free Meal Policy Statement - Must be filled out if Non-Charging for meals & attached to application
- Meal Count Consolidation Enrolled Sites Description - Attach if Enrolled Site
- Meal Count Consolidation Open Sites Description - Attach if Open Site
- Monitor Schedule Form - Must be filled out yearly & attached to application
- News/Public Release Enrolled Sites & Camps - Must be completed and attached to application
- News/Public Release Open Sites - Must be completed and attached to application
- Parent Application - If this pertains to your program- Must use this application
- Permanent Agreement - Must be filled out for Fiscal Year 2023 & uploaded
